two weeks into our west coast heat wave,
i got called to look at an outlet which was described as " melted and glowing with smoke coming out"

heh

the room was dark and it looked like there was an LED light on it,
doh!

that circuit had been cutting in and out for a week they said. acting as if the breaker had tripped, but checking the panel it hadnt. and then after a couple minutes the power would come back on.

as you can see in the last picture,
the heat cooked the insulation off the hot wires several inches back along their length. had to carefully crack and break this ruined insulation off working back along the wire hoping to reach decent insulation [ still plyable that is ] before there wasnt enough wire let to make a connection in the box. there was just enough left for that.

here is the fix.

decided not to replace an outlet in the location. the homeowners suggestion and i agreed. it would have been quite difficult to get a third wire into a connection with those short hot wires. not impossible. but very difficult.

now its just waiting for a blank cover plate. or for someone to get highly motivated and reinstall an outlet there. had the insulation been bad another half inch further back those wires, it would have required breaking out the wall board to either move that box or install another one near it for a new junction to allow the extension of the short wires.

How did this happen, was the load going through the plug and it just started to get a little thin.
